Language Interpretation: Medical Associates Health Plans has the ability to provide language interpretation for members who do not speak the English Language. IMPORTANT HEALTH COVERAGE TAX DOCUMENTS

Per federal law, health plans are not required to mail Form 1095-B for the reporting period. Therefore, you will not receive a Form 1095-B via postal mail. To obtain a copy of your Form 1095-B, contact Medical Associates Health Plans. Form 1095-B will be forwarded to you within 30 days of the date the request is received.
