At Medical Associates, we utilize a team of highly trained Population Health professionals including case managers, health coaches, and pharmacy specialists who work with local providers to manage costs and ensure the best patient experience. Medical Associates Health Plans includes all of these services with their coverage.
Health Coaching
Our health coaches are real people guiding members into healthy, sustainable behavior change. Resources are available through My eLINK to help you find appropriate, authoritative health information. You will find an illustrated medical encyclopedia, a medical dictionary, interactive health tutorials, and the latest health news. Recommendations and guidelines for immunizations, cancer screenings, blood pressure checks, blood tests, etc. are available through My eLINK. Detailed guidelines are available here.
Disease Management Programs
Education and support to help those with diabetes and hypertension. The programs include patient education, guidelines for care, recommended lab schedules, and phone contacts with the designated disease management nurse. Through education and monitoring, we attempt to effectively promote patient compliance and healthy lifestyles.
Case Management
Our case management nurses serve as a go-to person to help answer questions and serve as your advocate during episodes of care. Our nurses are locally based, they are your neighbors and your friends. They have a robust knowledge of the community and the healthcare services available and are here to help you navigate during these trying times. They focus on making sure you do not get lost as you work your way through the system and transition back to everyday life. We want you to focus on your health and getting well and let us facilitate the process.
